Fees and Payments

Payment is requested at the time of each visit. Cash, check, VISA, MasterCard, and American Express are accepted.

 

If an x-ray is ordered by your doctor, it will be sent to a radiologist. You will receive a separate bill for this service.

When lab work is ordered that is not performed in our office, the specimen will be sent to another location to be processed. You will receive a separate bill for this service.

Insurance

Assignment is accepted for most insurance, Medicare and Medicaid of Alabama. Please call ahead to verify insurance, and bring your identification cards with you. If you are covered by one of these plans, you will be asked to pay your co-pay or deductible at the time of your visit.

If we do not file your insurance, payment will be expected at the time of your visit. If you have been under the care and treatment of another physician, we will assist you in obtaining copies of your medical records, as well as a current list of prescribed medications. This will provide our office and staff with the information necessary to ensure that you get the very best medical care.
