
Group Term Life Insurance
Full-time (0.8-1.0) Employees:
- Basic Life insurance coverage in the amount of one (1) times your annual base salary to the next highest $1,000 is provided by SAMC for full-time employees (classified as a 0.8 or above and normally working 32-40 hours a week). Maximum benefit amount is $50,000.
- Voluntary Life insurance coverage may be purchased in the amount of one (1), two (2), three (3), or four (4), times your annual base salary to the next highest $1,000. Maximum benefit amount is $400,000. The guarantee issue amount is $50,000 at your first opportunity to enroll. Amounts over $50,000 must be approved per medical guidelines.
- Dependent Life insurance coverage may be purchased for your spouse and/or dependent children. Coverage is $10,000 for your spouse and $5,000 for each dependent child. Eligible dependents include a spouse not insured as a SAMC employee or in full-time military service; an unmarried child under age 19 (or under age 23 if a full-time college student), not in full-time military service, or covered as a dependent by another SAMC employee.
Part-time (0.5-0.7) Employees:
Voluntary Life insurance coverage may be purchased in the amount of one (1) times your annual base salary to the next highest $1,000. Maximum benefit amount is $400,000.
