Dental Coverage - BCBS of Alabama
Preferred Dentists will file all claims for you and accept the BCBS payment as payment in full (after any deductibles and co-pays). Non-Preferred Dentists may bill you for any difference between what BCBS pays and their billed amount. You may also be required to pay the dentist at the time of service and file the claim yourself.

 

 

Maximum Benefits per Calendar Year:

• $1,000 per member

Deductible:

• $50 deductible per person per year; 3 per family

Charges covered at 100%, no deductible or co-pay:

• Dental examinations (twice per year)
• Dental x-rays (twice per year)
• Dental cleanings (twice per year)
• Sealants (children under 14)

Charges covered at 80% after $50 deductible:

• Fillings
• Simple extractions
• Dental repair
• Oral Surgery

Charges covered at 50% after $50 deductible:

• Dentures, Bridges, Crowns

Orthodontic services:

Covered at 50% up to a $2,000 lifetime maximum
