
General Information
- Coverage becomes effective the 1st day of the month following your hire date or status change date.
All enrollment forms must be turned in to the Human Resources Department prior to the 1st of the following month in order to begin coverage. Otherwise, the annual open enrollment period (usually in October) will be your next opportunity to apply for coverage. Return all forms even if you are not electing coverage. - Benefit changes (additions/cancellations and/or increases/decreases) can be made only during Open Enrollment or if a "Qualifying Event" occurs. For example, marriage, divorce, birth of a child, adoption, loss/gain of other coverage, status change (PT-FT), etc. YOU MUST NOTIFY HR OF THE EVENT. Changes must be made within 30 days of the event. Proper documentation is required to make changes.
